The correct version of this is out there online, too, but there are a lot of incorrect ones, too. Here’s what I think it is.

Chords are named according to pitch but frets are given relative to capo.

During the piano solo it sounds like he changes up the chords slightly, removing the high C# that’s been a constant throughout the rest of the song (except in the C#7) and playing this instead. He may finish out the song with the new chord forms, as far as I can tell.
